The Bridges Program is a pre-hospice program consisting of non-medical supportive services and encouragement for Queen Anne’s County residents who have been diagnosed with a life-limiting illness, and who are not ready or appropriate for hospice care.
Bridges eligible individuals may still be receiving life-extending treatment and may benefit from practical, emotional and spiritual support to help them and/or their caregiver cope with a progressively serious illness.

Bridges Nurse
The Bridges nurse is an invaluable resource for individuals and families who are not familiar with the practical aspects and physical demands of hands-on-care. The Bridges nurse conducts an initial assessment visit and, in conjunction with your primary physician, makes a determination of the appropriateness of Bridges services. Monthly follow-up visits are conducted thereafter to determine appropriateness for continued services. While the nurse can help with coordination of care with your other healthcare providers, she/he is not able to provide medical care or replace those services you already receive from your medical providers or home health agency nurse.
Bridges is a free service
We rely entirely on the generosity of the community we serve and receive no payment from Medicare or reimbursement from insurance companies.
Length of services
To ensure we are using resources effectively, the needs of each Bridges participant are evaluated on a regular basis. A Bridges participant has the right to discontinue services at any time simply by calling the office at 443-262-4100.
